trying to swap out an old fitting, is this one a straight adapter from 3/8 NPTF male over to an SAE 45 flare male?

It is a straight adapter with a 3/8 NPTF male on one end and an SAE 45 degree flare male on the other. The listed end sizes are 0.375 and 0.75.

does this thing connect a 3/8 NPTF male fitting to a SAE 45 flare, or am I reading the specs wrong?

You're reading it right. One end is a 3/8 inch NPTF male thread and the other is a 3/4 inch SAE 45 degree male flare. It's a straight adapter, so there's no bend or angle to it. That combination makes it useful when you're connecting pipe thread ports directly to SAE 45 flare fittings in the same line.
