trying to replace an old 90 elbow, what sizes are on this one?

For fitment, this adapter is an NPTF male on one end and an SAE 45° male flare on the other. The NPTF end is size 0.5, and the SAE 45° flare end is size 0.875.

Does this adapter go from a half-inch NPTF male thread down to a 45-degree flare fitting, or is it the other way around?

You've got it right on the first end. One side is a 1/2 inch NPTF male thread and the other is a 7/8 inch SAE 45 degree male flare. The 90 degree elbow body puts those two ends at a right angle to each other, which helps a lot when you're routing lines in tight spots. It's brass construction with Dura-Kote plating to hold up against corrosion.
