both ends on this thing are male JIC 37-degree flare - so what exactly does it connect, two female JIC fittings on a hydraulic line?

Correct setup. Both ends carry male JIC SAE 37-degree flare threads, so you'd be threading each end into a female JIC fitting on either side of the connection. It's a reducer-style straight adapter, stepping down from a 1-1/16-12 thread on one end to a 3/4-16 on the other.
