Both ends on this thing are JIC flare, but the middle port is NPTF -- what size is that NPT connection exactly?

The branch port in the middle is a 3/8-18 male NPTF thread, conforming to SAE J476. The two run ends are both 9/16-18 male JIC SAE 37-degree flare, built to SAE J514. So you've got two flare ends inline and one tapered pipe thread coming off the branch.

i’ve got a hose end that’s 9/16-18 JIC, is that what both straight-through sides are on this tee?

On this run tee, both JIC ends are male JIC SAE 37 degree flare with 9/16-18 threads, built to SAE J514. The third leg is different, it is male NPTF 3/8-18 per SAE J476.
