bought this for a repair job on some old black iron lines -- does the 3/8 size here refer to the female thread opening on both ends?

Both ends are female threaded at 3/8 inch, so it connects two male-threaded pipe ends of that same size. That makes it a standard straight coupling, no reducers, no adapters needed. As long as your existing pipe is 3/8 inch male thread, this drops right in.
