what's the thread setup on this elbow, one end hose barb and the other NPT or am i reading that wrong

You're reading it right. One end is a hose barb connection and the other is a 1-1/4-11-1/2 male NPTF thread, so it bridges a barbed hose fitting to a standard pipe thread port. The NPTF side follows SAE J476 spec. Both ends are 1-1/4 inch sizing, which makes it a straight-size 90 degree adapter rather than a reducing one.
