What's the width across the flats on this fittin'?

This adapter has a wrench flat of 27mm, so a 27mm wrench will fit it.

trying to match up an oddball fitting, what are the exact thread callouts on both ends of this adapter?

On side 1 it is a DIN tube male connection with an M24 x 1.5 callout in size S16. Side 2 is a BSPP male connection with a G 3/8 - 19 callout in 3/8 inch size.

adapter's got a M24 x 1.5 on one end and G 3/8-19 BSPP on the other -- that right? just want to make sure before i order

Correct on both counts. One end is a male DIN tube connection, M24 x 1.5 thread, S16 size. The other end is male BSPP, G 3/8-19, which is 3/8 inch. Both sides are male, so keep that in mind when planning your fitting arrangement.
