What size threads are on each end of this fitting, and does it matter which end goes where when you're hooking it up?

One end carries a 1-1/2-11-1/2 male NPTF thread and the other end has a 1-1/4-11-1/2 female NPTF thread. Because those two sizes are different, orientation does matter. The male end connects into a female port on your system, and the female end receives a male fitting from the smaller line. Both threads meet SAE J476, so as long as your ports and fittings are NPTF and sized to match, the connection should seat properly.
