got a tight spot on a line, is this elbow the kind with female pipe threads on both ends?

You are looking at a 45 degree forged steel elbow with female NPTF threads on both ends. Both ports are 1/8-27 per SAE J476, so it is meant to join two matching male NPTF ends while turning the run 45 degrees.

Will this fitting work with standard NPT fittings, or does it need something special?

Correct on both counts. Each end carries a 1/8-27 female NPTF thread, and both conform to SAE J476. So if you're threading male NPTF fittings into each side, you're set up right.
