trying to hook a 3/4-16 male ORB port to a 3/4-14 female npsm swivel, is that what this adapter does?

For tying those two thread styles together, this piece is set up with a male SAE O-ring boss on one side (3/4-16, SAE J514) and a female NPSM on the other (3/4-14, SAE J476). It also uses an NBR O-ring on the ORB end for sealing.

what thread type does the male end on this adapter use, and is it got an o-ring built in or do I need to supply my own seal?

The male end is a SAE O-ring boss thread, sized 3/4-16 and built to SAE J514 spec. The o-ring comes with the adapter, it's NBR material, so no separate seal is needed on that end. That's one of the reasons the ORB style connection is popular for hydraulic work, the seal is already part of the fitting.
