fitting got a #10 male JIC flare end and needs to go into a #12 ORB port at a 45 degree angle -- that what this one does?

that lines up exactly. One end is a #10 MJIC, which is a male 37-degree flare fitting, and the other end threads into a #12 male O-ring boss port. The 45-degree angle is built into the body of the fitting itself, so you get that directional offset without needing a separate elbow or adapter in the line.
