got a hose end that’s 1-1/16-12 ORB, will this elbow screw right into it and still seal up?

Both ends are SAE J514 O-ring boss with 1-1/16-12 threads. Side one is a male SAE adjustable O-ring boss, and side two is a female O-ring boss, so it’s meant to mate with matching ORB ports and seal with the included NBR O-ring.

Both sides on this thing are 1-1/16-12 threading, does that mean it'll connect two ORB ports of the same size, or is there something different going on between the male and female ends?

Both threads are indeed 1-1/16-12 and both meet SAE J514, but the two ends serve different roles. Side one is a male SAE adjustable O-ring boss, which means it threads into a female ORB port and can be repositioned to get your hose or line pointing the right direction before you lock it down. Side two is a fixed female O-ring boss, so it accepts a male ORB fitting on whatever you're connecting to it. Same thread size on both ends, just opposite configurations with different functions.
