fitting's got a swivel on the female end, does that mean I can spin it into place without having to twist my hose line during install?

That's what the swivel is there for. The female NPSM end rotates freely so you can thread it onto your hose fitting and work it into position without putting any twist into the line itself. Makes a real difference in tight spots where you don't have a lot of room to maneuver. The male ORB side is the one you're threading into the port, and that end is adjustable, so you can clock the elbow to the angle you need before locking it down.

got a hose end that’s 3/8-18 NPSM swivel. will this elbow screw onto it and still give me a #10 ORB on the other side?

You can use it for that mix. One end is a #6 female NPSM swivel thread at 3/8-18 (SAE J476), and the other end is a #10 male SAE adjustable O-ring boss at 7/8-14 (SAE J514). It is a 90 degree elbow, so it also turns the line while making that thread conversion.
