Does this adapter go from a JIC fitting on one end to a metric ISO port on the other, or am I reading the thread specs wrong?

You're reading it right. One end is a 7/8-14 male JIC with a 37-degree flare, which follows SAE J514. The other end is a male M22x1.5 metric fitting that seats into an ISO 6149 O-ring port. So yes, it bridges those two different thread standards in a single straight adapter. The O-ring seal material is NBR, which handles a good range of fluids well in typical hydraulic setups.

trying to swap out an old fitting, is this one the straight adapter that goes from 7/8-14 JIC flare over to an M22x1.5 ISO 6149 o-ring port?

Function-wise it is a straight adapter with a male JIC SAE 37 degree flare on the B1 side in 7/8-14 (SAE J514) and a male metric ISO 6149 O-ring connection on the B2 side in M22x1.5. It uses an NBR O-ring seal on the ISO 6149 side.
