What's the deal with the different sized ends on this thing?

It's made with a JIC (Joint Industry Council) end on one side, which is 5/16" size and 1/2 - 20 UNF callout, and a Bspt (British Standard Pipe Tapered) end on the other side, which is 1/4" size and R 1/4 - 19 callout. That lets you connect different kinds of hydraulic fittings together.

got a hose end that’s 1/2-20 UNF JIC, will this adapter match up to it?

On the JIC side, it’s a male 5/16 inch with a 1/2 - 20 UNF thread callout, so it matches a 1/2-20 UNF JIC connection on that end.

Got a line running R 1/4-19 BSPT on one end and JIC on the other, this the right adapter to tie them together?

That's what this one's built for. One side is male JIC with a 1/2-20 UNF thread at 5/16" size, and the other side is male BSPT at R 1/4-19 with a 1/4" size.
