fitting's got a 3/8 male end and I need to connect it to a 1/2 inch female BSPP port, is that what this thing does?

That's what it handles. One end is a 3/8 inch male BSPP thread (G 3/8-19) and the other is a 1/2 inch female BSPP (G 1/2-14), so you're getting both a size transition and a direction change in one piece. Saves you from stacking an extra adapter in the line.

trying to swap a fitting on a tight hose run, what threads and sizes are on each end of this 90 elbow?

On the male end you get a 3/8 inch BSPP thread, called out as G 3/8-19. The female end is 1/2 inch BSPP, called out as G 1/2-14, and the body is a 90 degree elbow.

Reckon this'll hold up okay in a hydraulic setup on a farm tractor? Just need somethin' to change the direction of the line a bit.
