Will this fitting work with regular NPT threads, or is it strictly for BSPT?

This elbow has a male BSPT connection on one side, and a male JIS connection on the other; it's not compatible with NPT threads.

Got a piece of equipment running JIS fittings on one end and BSPT on the other, does this elbow handle both those thread types in the same fitting?

That's what it does. One side is male JIS with a G 1/2 - 14 thread, and the other is male BSPT with an R 1/2 - 14 thread. Both sides are 1/2" size, so you're getting a direct 90-degree connection between those two standards without needing a separate adapter in between. Steel construction on the whole thing.
