trying to swap out an old elbow fitting, is this one 1/4-18 female npt on both ends?

Both ports are 1/4-18 female NPTF, so it is a female to female 90 degree elbow adapter.

Both ends are female NPT on this one, right? Trying to route a line around a corner and need female threads on each side so I can run two male-threaded fittings into it.

Correct, both ends are 1/4-18 female NPT, so you can thread a male-fitting into each side and make that 90-degree turn without needing any kind of union or extra adapter in between. The elbow gives you a clean right-angle change of direction, which works well when space is tight and you need to keep the line tucked close.
