Does this adapter go from pipe thread on one end to a 45 degree flare on the other, or are both ends the same type?

One end is 1/2-14 NPT male pipe thread and the other is 3/4-16 male SAE 45 degree flare. So yes, you're bridging two different thread types with this one piece. It's a straight body, no angle to it, just a direct inline connection between those two standards.

got a line with 1/2-14 male npt on it, will this adapter screw right on and give me a 3/4-16 male sae 45 flare on the other end?

For a straight thread changeover like that, yes. One side is 1/2-14 male NPT (pipe thread) and the other side is 3/4-16 male SAE 45 degree flare, so it’s meant to transition between those two connection types in a straight adapter.
