got a 1/8-27 npt port on a gauge and need to land a 1/4 inch flare line, is this the right adapter?

For that hookup, it matches up as a straight pipe to 45 degree flare adapter with a 1/8-27 male NPTF on one end and a 7/16-20 male SAE 45 degree flare on the other. It is also listed with a 0.25 inch tube OD for the flare side.

does this adapter go from NPT pipe thread on one end to a 45 degree flare fitting on the other, or am I reading the specs wrong?

You've got it right. One end is 1/8-27 male NPT and the other is 7/16-20 male SAE 45 degree flare. Both ends are male, so you'd be threading this into female NPT on one side and connecting to a female SAE 45 flare on the other. It's a straight adapter, no bends or angles to it.
