fitting says 0.38 inch hose ID, does that mean it'll work on a standard 3/8 inch hose, or is there some difference I'm missing?

Same size. The 0.375 inch hose ID listed in the specs is just the decimal version of 3/8 inch, so if your hose has a 3/8 inch inside diameter, this fitting is sized right for it. The shank slides into the hose end and gets secured with clamps, same as any standard shank-type setup.
