putting this on an A/C line in a tight spot under the dash -- does the 90 degree elbow actually give you enough room to work, or is it still a bear to get in there?

Tight underdash work is exactly what that 90 degree configuration is built for. The elbow redirects the hose run so you're not fighting a straight fitting in a corner where there's no clearance. The E-Z Clip system itself helps too since you only need the FT1357 Clip Closing Pliers to finish the connection. No crimping machine, no power source. As long as you cut the hose square, get both clips slipped on before seating the nipple, and make sure there's no gap between the hose end and the nipple shoulder, it goes together pretty clean even in confined areas.

trying to hook up a -10 hose, what all does this fitting actually connect to?

On the hose side it’s made for a -10 GH134 Multi-Refrigerant hose using the E-Z Clip setup with a cage and two clips. On the other end you’ve got a 7/8-14 female SAE 45 degree swivel connection, and the fitting body is a 90 degree elbow for routing in tight spots.
