got a 3/8 JIC line on a hydraulic system and need to tie into a DIN 2353 light series port sized at M18 x 1.5 -- this adapter do that in one piece or am I gonna need something extra?

One piece is all you need here. One end is a 3/8" Male JIC with a 9/16-18 UNF thread, the other is an M18 x 1.5 Male DIN 2353L fitting, which is the light series L12. Straight configuration, no intermediate hardware required. At 38mm overall length it stays pretty compact in tight hydraulic connections situations.

Will this here fitting hold up if I'm usin' it outside where it gets rained on a lot?

Because it's made from 316 stainless steel, this adapter offers good corrosion resistance, so moisture shouldn't be a problem.
