Both ports are female NPT on this thing, so what exactly goes in between them? Like do I need two male nipples to make it work in a line?

Yeah, you'd run a male-threaded nipple or fitting into each port. Since both ends are 1/4-18 female NPT, you're connecting two male-threaded components together at a 90-degree angle.
