trying to tee off a line, is this one got a female swivel on the branch and a male on the run like it says?

Function-wise, it is a tee adapter that gives you a 7/8-14 female JIC 37 degree flare swivel on the branch, and a 7/8-14 male JIC 37 degree flare on the run, so you can split or tap into a JIC 37 degree flare line at that size.

what exactly is the swivel side on this tee for, does it let you rotate after it's tightened down or is it just for alignment during install?

The swivel on the female JIC end is there to help you get the fitting lined up without having to twist the hose or tube it connects to. Once everything is torqued down it locks in place, so it's not meant to spin freely in service. The real value is during installation, especially in tight spots where you can't rotate the whole assembly to get the threads started right.
