got a tight spot on a hose run. is this the 90 degree elbow that goes from 1/4-18 male npt over to 3/4-16 male jic 37 flare

For a right angle turn, it’s a 90 degree elbow adapter with 1/4-18 male NPT on one end and 3/4-16 male JIC with a 37 degree flare on the other.

What kind of fitting is on each end of this adapter, and how does the elbow come into play?

One end is a 1/4-18 male NPT thread and the other is a 3/4-16 male JIC 37 degree flare. The 90 degree elbow angle means the two ports point in perpendicular directions, so instead of running a line straight through, you can turn a hard corner right at the fitting. That comes in real handy when you're tight on space or need to keep a hose from bending awkwardly behind a panel or near a bulkhead.
