trying to swap a straight ORB to JIC elbow on my line, are the threads on this one 1-1/16-12 on the ORB side and 3/4-16 on the JIC side?

The SAE O-Ring Boss end is 1-1/16-12 male straight thread, and the JIC 37 degree flare end is 3/4-16 male. It is a 90 degree elbow adapter that goes from SAE ORB to SAE 37.

got a port on a hydraulic motor that's 1-1/16-12 SAE O-ring boss and i need to run a JIC line off it at a 90 -- this the right adapter for that?

That's what this one covers. The male SAE O-ring boss end is 1-1/16-12 straight thread, and the other end is 3/4-16 male JIC 37 degree flare. The 90 degree elbow lets you turn the line without needing a separate street elbow or extra fittings in the run. Just make sure your port accepts a male ORB thread and your hose or tube end is a female JIC swivel, since both ends on this adapter are male.
