got a hose end that’s 12 JIC, will this adapter swap me over to ORB on the other side?

Function-wise, it bridges a 1-1/16-12 male JIC 37 degree flare on one end to a 1-1/16-12 male SAE O-Ring Boss straight thread on the other, so it is meant for going between those two connection styles at the same size.

Got a hydraulic line running from an ORB port on a valve block and need to tie into a JIC fitting downstream. this adapter go straight through without any angle or offset to it?

Straight through is exactly what you get. Both ends run inline with no bend or angle, so flow goes directly from the SAE O-Ring Boss port on one side to the 37-degree JIC flare connection on the other.
