trying to tee off a line, are all three ends on this adapter the same size or do i need reducers?

All three ports are the same. It is a steel tee with 11/16-16 male SAE O-Ring Face Seal (ORS) threads on the run and on the branch, so you would only need reducers if the rest of your hydraulic connections is a different ORS size.
