What exactly is the difference between the two thread ends on this adapter, and which side hooks up to which fitting in my system?

One end is a 1-11/16-12 male SAE O-ring face seal, and the other is a 1-5/8-12 male SAE O-ring boss, which is also called a straight thread O-ring. The face seal end seals against a flat surface using an O-ring on the face of the fitting. The O-ring boss end threads into a port and seals with an O-ring that sits in a groove behind the threads. Match each end to its corresponding port type in your system and you'll get a solid, leak-free connection without any thread sealant needed.
